Case Management

The IHS Case Management services provide personalized coaching and ongoing support to help individuals and families achieve stability and independence.

  • Case managers assist clients in accessing housing, financial benefits, healthcare, employment, and behavioral health services while coordinating care with community partners.
  • Through general, community-based, family, and veterans case management, services are tailored to address mental health and substance use needs, strengthen family systems, support veterans, and build the skills necessary for successful community living.

  • The Case Management Program at CER Hawaii (Community Empowerment Resources) is a state-funded service (through Hawai‘i’s Adult Mental Health Division) that supports adults with serious mental health needs by helping them maintain psychiatric and medical stability, access benefits, secure and keep housing, coordinate legal issues, build daily living skills, grow personally, improve relationships, participate in community activities, and work toward greater independence.
  • Case managers work closely with clients to connect them to the right supports and services and help them stay stable and engaged in their community.

  • HMSA’s Complex Case Management Program is a voluntary, no-cost service for eligible members who have complex or serious health conditions and need extra support managing their care.
  • Hawaii-based registered nurse case managers work closely with members, caregivers, and doctors to coordinate treatment, medications, and services, ensuring care is timely and well organized. 
  • The program helps members understand their health conditions, follow care plans, set health goals, prepare for hospital discharges, access community resources, and plan for changes in their health, all to improve overall health and well-being.

  • The Hawai’i Health & Harm Reduction Center (HHHRC) offers comprehensive case management services to marginalized populations in Hawai’i

  • Hawai’i Health and Harm Reduction 
  • Care Hawaii’s Community-Based Case Management (CBCM) / Community Care Services (CCS) is a support program for adults with serious mental illness who need long-term, community-based assistance. It helps members stay stable in the community by providing ongoing case management, coordinating mental health and medical services, and connecting them to housing, benefits, and supportive resources. The program focuses on improving daily living skills, reducing psychiatric symptoms, and preventing hospitalization or homelessness, while supporting long-term recovery and independence.

  • Family Programs Hawaii’s Voluntary Case Management (VCM) program serves families on Oʻahu who are referred by Child Welfare Services after a report to the Child Abuse and Neglect Hotline. 
  • Families identified as moderate to moderate-high risk are assigned a case manager who works with the entire family to address challenges that place children at risk of abuse or neglect. 
  • The goal of the program is to strengthen family functioning, improve safety and stability, and help families remain together while preventing entry into the foster care system.

  • Rainbow Health Hawaii’s Case Management — Community Integration Services (CIS) is a Medicaid‑funded program that helps people experiencing or at risk of homelessness in Hawaiʻi obtain and keep stable housing while connecting them with coordinated support services.
  •  Under CIS, trained case managers work one‑on‑one with eligible QUEST Integration members to navigate housing applications and landlords, link clients to healthcare, behavioral health, and social supports, and provide pre‑ and post‑tenancy coaching and advocacy to build daily living skills and promote long‑term stability in the community.

  • Mental Health Kokua’s Case Management Program provides individualized support for adults with serious mental illness, focusing on improving self-sufficiency and helping clients live independently in the community. 
  • Case managers work with clients through assessment, planning, coordination, advocacy, and monitoring to connect them with needed services such as housing, mental health care, and substance use treatment.
  • The program supports clients in achieving personal goals, maintaining stability, and accessing community resources to promote long-term recovery and independence.

Disability

ADA Accommodations (via Kokua ‘Ike)

Steps:

  1. Fill out the Accommodations Request Form
    a. You need to submit documentation that describes your disability and its functional impacts;
    b. BUT you may submit the form and set up a Welcome Meeting without documentation 
  2. You will be contacted to set up a Welcome Meeting
    a. You will discuss your disability(ies) and the accommodations you are eligible for
    b. If you do not have documentation, you will discuss documentation needs
  3. If you’re eligible for accommodations, Kokua Ike will prepare and send an agreement for you to review

If you are diagnosed with a qualifying disability, it will fulfill the documentation requirement for accommodations.

Assessment Center

  • Provides free comprehensive psychological assessments to students
  • Testing may take 4-6 hours (this can be broken up into multiple sessions ex: 4 one hour sessions)
  • Final assessment reports will include test results, diagnostic impressions, and recommendations 
  • Schedule an appointment by emailing [email protected] 
    • Include your name, contact information, reason for appointment, and availability for testing session(s)
  • Hours of operation: Monday – Friday, 8:30 AM – 4:30 PM

ADA Parking

Students with accessible parking requirements may request a reserved parking space

  • The request must include a copy of the requestor’s Blue Card
  • A current ADA Parking Permit is required to use the space.

ADA parking stalls are located in the lot adjacent to the Mystical Rose Oratory.

  • The Workforce Innovation and Opportunity Act (WIOA) is a federal program that helps people to get the education and training they need to find a good job – especially in high-demand careers. WIOA funding can cover tuition, training fees, and even support services like transportation or childcare.

Honolulu Community Action Program – Na Lima Hana: 

  • Nā Lima Hana “for the working hand” Employment Core Services Program provides personalized one-on-one employment counseling services, job readiness training,  tuition assistance for vocational school, and job placement to unemployed and underemployed individuals throughout the island of O‘ahu.

Housing & Basic Needs

Residence Life & On-Campus Housing

On-campus emergency housing may be available within the residence halls and is considered on a case-by-case basis, depending on availability and approval from the Director of Residence Life & Housing.

Student Emergency Fund

Students may submit an application for a grant once they have exhausted all other resources

Eligibility requirements (must meet all)
1. Currently enrolled, full-time Chaminade students (undergraduate, graduate, online)
2. Immediate and unexpected financial hardship that has a direct impact on student’s academic success at CUH
3. Resources must have been considered either insufficient, unavailable, or not available in a timely manner 
4. Applicant must be able to provide appropriate documentation to verify circumstances for the funding request
a. Any supporting documents (bills, etc.) must be in the student’s name

Any funding they receive does not need to be paid back
1. One award will be made to any one student during all combined enrollments at CUH
2. Awards will be up to $1000

Eligible expenses that may be covered (this list does not cover all eligible expenses)
1. Medications and other costs related to emergency medical care
2. Books and other essential academic expenses
3. Safety needs
a. Includes the needs of victims of domestic violence
4. Replacement of essential personal belongings or temporary housing needs, due to fire, theft, or natural disaster
5. Travel costs related to death or illness in immediate family
6. Overdue essential utility bills reaching a turn-off notice
7. Homelessness or sudden loss of housing
8. Food insecurity
9. Loss of childcare
10. Assistance with rent or utilities due to an emergency situation


Expenses that are not covered (this list does not cover all ineligible expenses)
1. Tuition payment, fees, health insurance, study abroad costs, meal plan, residence hall costs
2. Non-essential utilities, household, or furniture costs not related to damage or theft
3. Parking citations or library fines
4. Costs for entertainment, recreation, non-emergency travel or other non-essential expenses
5. Legal fees, penalties, jail bonds
6. Other anticipated expenses

Section 8 Housing

A section 8 housing choice voucher that helps low income families, seniors, and disabled folks pay rent for private housing. It is provided by the Department of Housing and Urban Development (HUD), and managed by your local public housing agency (PHA)
