Get Started with Our Services
Students New to Accessibility Services
Step 1: Complete the Request Form
If you have documentation of your disability, you may upload it through the Request Form or submit it by email, mail, or in person. If you do not yet have documentation, that is okay. Complete the Request Form and book an appointment with the Accessibility Services Coordinator. We prioritize listening to students and will discuss any documentation needs during the initial intake meeting.
Step 2: Book an Appointment with the Accessibility Services Coordinator
Visit Stellic and sign in using your Chaminade ID and password. Once signed in, locate the Quick Links section. Click Appointments, then click Schedule. Under Host, enter the name of the Accessibility Services Coordinator, referring to the Office of Student Success Contact Us page if needed. Select a session type, date, and time, then click Schedule to confirm your appointment.
If you cannot book an appointment through Stellic, including if you are a newly admitted student and do not yet have full account access, please contact the Accessibility Services Coordinator at [email protected] or (808) 735-4733. We will assist you directly and ensure you receive the support you need.
Students Returning to Accessibility Services
If you have already registered with Accessibility Services, you may book an appointment with the Accessibility Services Coordinator to review your accommodations, discuss any updates, or address any questions or concerns. Appointments may be booked through Stellic using the steps outlined above.
Please note that all accommodations expire on July 31 each year and must be renewed annually. To renew, all students must re-submit the Request Form. If you will continue with your current accommodations and do not need any changes, a meeting with the Coordinator is not required as part of the renewal process. However, if you would like to request adjustments or additions, a meeting with the Coordinator is required.
Appointments may be held in person or virtually. Drop-in appointments are welcome during business hours, though scheduled meetings are encouraged to ensure availability.
